Lake County, CA – CoC and HMIS Capacity Building (CA-529)

From 2019 to present we've worked with Continuum of Care CA-529, Lake County CA to procure, implement and train the community on Social Solutions Group, Apricot 360. Our work encompasses implementation management, Coordinated Entry Design, Emergency Housing Voucher program design, referral workflows, role assignment, end-user training and reporting designed to oversee a diverse allocation of the EHV placements.

We worked with with the CoC to design a custom vulnerability assessment and WEbi reporting  for CES Prioritization, and we accompanied Lake County during Cohort-3 of the Rapid Results Institute creating weekly reporting to manage to client placement targets.

Our capacity strengthening approach for the CoC, has been reliant on the learning principles of SAP Design Thinking models  and Acumen.org, a nonprofit working to changing the way the world tackles poverty and building a world based on dignity.
