Alameda County, CA Health Care Services Agency – Data Exchange Unit

Decipher works Alameda County Data Exchange Unit (DEU) to integrate Clarity HMIS data with a new Social Health Information Exchange and Community Health Record (CHR) platform designed in Syntranet, an integrated health care and services coordination platform by Thrasys.

A one of it’s kind platform, the SHIE in two short years has combined 50 local leaders from eleven organizations to store data on 18,000 plus high-utilizer medical and housing records for the purpose of connecting service providers into an integrated whole person care service approach.  The CHR, an end-user user interface, allows service providers a highly targeted, timely slice of client utilization, diagnosis and promotes partner support.

Our role on the DEU was to provide HMIS data literacy training for CHR programming team, create HMIS specific data dictionaries and work internally with Alameda Care Connect Housing staff to design, track and report Whole Person Care metrics being recorded in BitFocus Clarity.

We also worked with the Home Stretch business unit to design a permanent supportive housing Unit matching database in Microsoft Access. This boutique solution imports Alameda County Coordinated Entry By Name List data and related client HMIS programs history, to automate referrals based on eligibility matching to permanent supportive housing units.
